Hospice Palliative Care Fellowship – How to Apply

Candidates must be graduates of approved family practice or internal medicine residency programs. Fellows must be board certified in Family Medicine or Internal Medicine and obtain medical licensure in the state of North Carolina. The one-year fellowship begins July 1.

Appy via ERAS®, or download our application form (PDF), or from the office of the fellowship director. Along with a completed application form, please submit the following:

  • A current curriculum vitae
  • A brief statement of career goals in geriatrics and family or internal medicine
  • Medical School Transcript (if more than one Medical School, need all transcripts)
  • Medical School Deans Letter (MSPE) (starting/end date and type of degree)
  • Three letters of reference: including the residency program director
  • USMLE/COMLEX Step 1, 2 CK and 2 CS Score report (for PGY level 1 and 2)
  • USMLE/COMLEX Step 3, Score report (for PGY level 3 and above)
  • ECFMG Certificate/Status Report (if a Foreign Medical Graduate)
